TERMS AND CONDITIONS

In consideration of participation in Higher Intentions basketball training programs and use of facilities and equipment associated with program activities, the participant and/or parent or legal guardian of a minor participant (“Participant”) agrees to the following:

1. Assumption of Risk

I understand that participation in basketball training and related athletic activities involves inherent risks, including but not limited to falls, collisions, contact with other participants, equipment-related injuries, and the potential for serious bodily injury, illness, permanent disability, or death. I voluntarily assume all such risks on behalf of myself and/or my child.

2. Waiver and Release of Liability

I hereby release, waive, discharge, and agree not to sue Higher Intentions, its owners, coaches, employees, volunteers, contractors, affiliates, sponsors, and representatives (collectively, the "Released Parties") from any and all claims, liabilities, demands, actions, causes of action, damages, costs, or expenses arising out of or related to participation in Higher Intentions programs, including those resulting from ordinary negligence to the fullest extent permitted by law.

3. Medical Authorization

In the event of injury or medical emergency, I authorize Higher Intentions staff to obtain emergency medical treatment if a parent/guardian cannot be reached. I understand I am responsible for all associated medical costs.

4. Participant Health & Medical Responsibility

I certify that the participant is physically able to take part in basketball training activities. I understand it is my responsibility to disclose any medical conditions, allergies, or limitations that may affect participation.

5. Photo and Video Release

I grant Higher Intentions permission to photograph and record participants and to use such content for training, educational, promotional, marketing, and social media purposes unless otherwise noted in writing.

6. Code of Conduct

Participants are expected to demonstrate respect, effort, accountability, and sportsmanship at all times. Higher Intentions reserves the right to suspend or remove any participant whose behavior is disruptive, unsafe, or inconsistent with program standards.

7. Attendance and Participation Expectations

Consistent attendance is expected for all practices, training sessions, and tournaments. Absences may impact development opportunities, team roles, and participation within the program.

8. Team Placement and Evaluation

Registration and payment secure participation in the Higher Intentions program. Final team placement (when applicable) will be determined through evaluations and is based on skill level, developmental needs, competitiveness, and overall roster balance.

9. Independent Program Notice

Higher Intentions is an independent basketball training organization and is not affiliated with, sponsored by, or acting on behalf of any school, school district, athletic department, or interscholastic athletic program. Participation in Higher Intentions activities is voluntary and is not connected to team selection, roster decisions, playing time, player evaluation, placement decisions, or any other personnel decisions for any school basketball program.

10. Payment and Refund Policy

Program fees are required prior to participation unless otherwise approved. Unless otherwise stated, all payments are non-refundable once roster placement has been confirmed or after the published refund deadline. Higher Intentions reserves the right to adjust schedules, locations, or training formats when necessary. Canceled sessions may be rescheduled when applicable.

11. Personal Property

Higher Intentions is not responsible for lost, stolen, or damaged personal property brought to any program activity.

12. Acknowledgment and Agreement

By registering and participating in Higher Intentions programs, I acknowledge that I have read, understood, and voluntarily agree to these Terms and Conditions.

Electronic signature (typed full name) submitted through registration constitutes legal agreement to these Terms and Conditions.
